Order Information
All orders must sign Professional Disc Services Indemnity Form and pressed CD orders must also sign the IPR form. Orders will not be processed without all forms being signed and completed. Forms can either be faxed to us at 973-696-8529 or mailed to our mailing address below.

Shipping Your Order To Professional Disc Services
We recommend properly packaging your orders when sent to us. Your master CD is very delicate and should be packaged properly to ensure safe shipping and handling. Make sure to include the following when shipping your order:

• Master CD with Timesheet and Tracklist
• All Graphic's/Design work on CD-R, DVD-R or Zip Disk (if providing artwork)
• Graphic Design Proofs (if providing artwork)
• Order Confirmation form (Invoice) from Professional Disc Services Representative
• Indemnity Form
• IPR Form (For pressed CD's only)

Payment and Shipping Options
All prices do not include sales tax.  All orders are to be paid in full at the time order is placed. No orders will go into production without payment being received in full. Orders can paid via credit card (Visa, Mastercard, American Express). Money orders can be sent to our mailing address. Sorry, we do not accept personal checks. Shipping and handling is additional on all orders. We only ship via UPS or FEDEX. Your sales representative will explain all the shipping options currently available.
